Phyllis Forrest

Phyllis Ann Cook Forrest, 78 of Kannapolis, passed away on Saturday, August 18, 2012 at Carolinas Medical Center - Northeast. The family will receive friends at Hartsell Funeral Home of Concord on Wednesday, August 22, 2012, from 12:30 p.m. until 2:00 p.m. The funeral service, officiated by the Rev. James Strickland, will follow in the Hartsell Funeral Home Chapel at 2:00 p.m. Burial will follow at Carolina Memorial Park in Kannapolis. Phyllis was born on October 3, 1933 in Cabarrus County, to the late Lloyd David Cook and the late Annie Fink Cook. In addition to her parents, she was preceded in death by a son, Tony Ray Forrest. She is survived by two daughters, Lisa Forrest of Kannapolis and Cynthia Honeycutt of Concord; a son, Ray Forrest of Concord; seven grandchildren; and two great-grandchildren. Hartsell Funeral Home of Concord is serving the Forrest family.
